Remembrance of the Last moment determines the Afterlife - yaṃ yaṃ vāpi smaranbhāvaṃ tyajatyante kalevaram। taṃ tamevaiti kaunteya sadā tadbhāvabhāvitaḥ  You will attain the one to whom your mind is attached at the time of death. Hence, God told Arjun - 'tasmāt sarveṣu kāleṣu māmanusmara yuddhya ca' Always

Remembrance of the Last moment determines the Afterlife -

 You will attain the one to whom your mind is attached at the time of death. Hence, God told Arjun -

'tasmāt sarveṣu kāleṣu māmanusmara yuddhya ca'

Always keep your mind attached to me.

 Therefore, you must keep both death and God in mind because you do not know when you will die. If you forget about death, you will become careless, thinking, "I am certainly not going to die so soon."

 If you keep your mind focused on God "always," then death will come at some point within that "always." Thus, you will attain your goal and go to God's abode.

 It is the greatest wonder that people witness individuals of all ages dying before their eyes every day, yet no one thinks about their own death. Did the one who died think, "I will die at just twenty-five?" Even an eighty-year-old does not think so. If someone truly reflects on this, then the identifying characteristic of that person is 'sarveshu kaleshu' - they will lovingly remember God "at all times" and do nothing else, because only the remembrance of your final moment will be of help to you. At that final moment, wherever your mind dwells, that alone will determine your afterlife.
